All entrants (including technicians) must be from the UK and Republic of Ireland or GDC registered practitioners.
All entrants work must be predominantly performed in the UK to be eligible
All entries are judged anonymously – Remove your name and contact details from the entry itself including any reference to you in the patient testimonial, however, include them in an email/covering letter.
All entries must be electronic, with each entry’s case report as a Word document and photos separately sent using an online file sharing service or on a USB stick.
Only one entry per category per person.
The same patient must not be entered into multiple categories (A patient entered into one clinical category can also be submitted into one Patient Choice Award category).
Entries will not be returned.
The judges reserve the right to reallocate entries, if they feel it appropriate.
